The staff of Heart Specialists of Sarasota is happy to welcome you to our practice, and we thank you for choosing us for your cardiovascular care. We look forward to providing you with the latest advances in prevention, diagnostics, intervention and treatment of heart and vascular disease. This section of our website is devoted to new patients to our office so that the first meeting may be as comfortable, efficient, and productive as possible.
Schedule an Appointment:
To schedule an appointment at our convenient location, please call our main office at (941) 917-4250, between the hours of 9:00 a.m. to 12:00 p.m. and 1:00 p.m. to 4:00 p.m., Monday through Friday. Please have your calendar in hand and a list of possible dates and times that are convenient for you.
Before Your Appointment:
Please contact your physician(s), hospital, or medical facility to obtain your medical records prior to your appointment. For your convenience, we have included an “Authorization to Release Medical Records” form here on our website. Click to download
Please complete the new patient packet of information and bring it with you to your appointment. We feel that it is easier for our patients to complete this paperwork at home where they have documents to which they can refer and have family members available who can assist them. What to Bring:
- All current insurance card(s) and a photo ID
- Your completed “New Patient Registration Forms”
- Previous medical records, in your possession
- All current prescription medications(s) and supplements, in their original containers
Always bring your insurance card(s) with you to each and every office visit, even if you have had no change in coverage. Please bring all prescription medications and supplements in their original containers for our review. If this is not possible, a list that includes the medication/supplement name, dosage, and how many times a day you take it, is acceptable. The medication list should contain the exact name of the drug, dosage and prescription instructions.
What to Expect on Your First Visit:
Your visit may be telehealth or in person. If you don’t remember, please call our office to verify. If your visit is telehealth, you will receive a call from your nurse the day before with the process to check-in. Plan to arrive at least thirty minutes prior to your scheduled appointment time. Upon arrival please check in with a staff member at the front desk. This individual will verify your personal information, review your completed new patient forms, and scan your insurance cards. This information is carefully checked and verified at each and every appointment. Please be patient with our staff as it of the upmost importance that we maintain accurate patient information.
After the check in process is completed, you will be asked to have a seat in our waiting room until it is time for your appointment. We at Heart Specialists realize that your time is valuable, as is that of our doctors and staff. With that in mind, we make every effort to see our patients at their scheduled appointment times.
Heart Specialists of Sarasota understands that it may be necessary to reschedule an appointment due to an unexpected circumstance. We ask that you please be considerate of our provider’s time and call to reschedule 24 hours in advance to allow other patients to be seen at your assigned appointment time. Failure to provide 24 hour advanced notice will result in a $25.00 “no show” charge for an office visit and up to $150.00 for an advanced imaging test such as a Stress Echo, Nuclear Stress Test or PET.
